Gather Your Documents

To properly complete this questionnaire, you may need to upload PDF scans of the following documents:

  1. State issued driver license or ID card for each petitioner.
  2. State issued ID card for any child being adopted who is 12 years of age or older.

ADD IMAGE UPLOAD FOR EACH CHILD somewhere, not mandatory in case they don’t have one

  1. A copy of your marriage certificate if you are married.
  2. A copy of the birth certificate for each child being adopted.
  3. Social Security numbers for each child being adopted.
  4. Copies of final judgements or court orders from related cases such as custody, child support, etc.

*If you don't have the above documents handy at this time, you can still complete this questionnaire.  However, you must e-mail these to us as soon as possible.

COURT FILING FEES

We will prepare the application(s) for you, at no additional charge, for consideration by the Clerk of Court.  The Clerk of Court makes the final decision on whether to approve the application(s) and waive your court filing fees. If you and your spouse are adopting together, BOTH of you must complete a separate APPLICATION FOR FILING FEE WAIVER.

Options for Signing and Notarization of Documents

REMOTE ONLINE NOTARIZATION (RON)

We have certified online notaries on staff.  If you qualify to use this system, you have the option of signing your relative adoption documents from home using your smartphone or computer/webcam without having to make a trip to a notary public and mail back paper copies.  

HOW MUCH DOES IT COST?

We are currently offering our remote online notarization for $30 per signer for all initial documents prepared for the petitioner(s) by Florida Family Adoptions for this relative adoption project. 

Please Note: Documents signed by consenting birth parents and/or anyone other than the petitioner(s), are an additional cost

Minimum Requirements

  • System Requirements
  • Desktop or laptop computer, tablet, or smart phone with a working camera and microphone.
  • A reliable, high-speed internet connection.
  • Up-to-date web browser and operating system. 
  • Each signer must have their own email address.
  • Signers must be conversant in the English or Spanish languages, or use our foreign language translator service.

Accepted forms of photo identification:

  • U.S. Passport
  • A Passport from Any Country
  • Florida Driver License or ID Card
  • Driver License or ID Card Issued by any U.S. State or Territory
  • Driver License or ID Card Issued by Canada or Mexico
  • An identification card issued by the United States Bureau of Citizenship and Immigration Service
  • U.S. Uniformed Services ID Card

Photo identification must be current or have been issued within the past five (5) years.

I confirm that I understand if I/we change our mind(s) about using RON for signing or are unable to complete the ID Verification and/or any part of the RON session AFTER all documents have been completed, that the documents would then need to be revamped. The revamping of the documents could could take up to a couple of weeks to complete and I may be charged an additional fee for this change. *

DISCLAIMER ON DOCUMENT PREPARATION

PLEASE NOTE: We will NOT begin preparation of your adoption document packet until we have ALL required documents, information and/or research is completed, which could include the following:
1. Copies of all documents requested in this questionnaire.
2. Court filing fee waiver application(s) are submitted for each petitioner on this adoption case, if applicable.
3. 50% deposit on your invoice is received and recorded in our system.
4. Disclosure packets from Florida Family Adoptions is signed by all required parties. * You will be assigned a Document Specialist once you have completed your deposit & signed our disclosure packet.
5. Questionnaire review and approval has been completed by you and by your assigned Document Specialist(s).
6. Any missing information and/or modifications needed on your questionnaire that are requested by you and/or your assigned Document Specialist(s).
7. Copies of any additional documents required and/or requested by your assigned Document Specialist(s).
8. All county specific requirements are reviewed and researched, as needed, by your Document Specialist(s). *
