Gather Your Documents

To properly complete this questionnaire, you may need to upload PDF scans of the following documents:

State issued driver license or ID card for each petitioner.
State issued ID card for any child being adopted who is 12 years of age or older.
A copy of your marriage certificate if you are married.
A copy of the birth certificate for each child being adopted.
Social Security numbers for each child being adopted.
Copies of final judgements or court orders from related cases such as custody, child support, etc.
