Athlete Protection:

If selected to serve on a Sport Development camp or a National Team camp/trip, USA Swimming National Office Athlete Protection Policy requires that you successfully complete additional screening prior to being formally offered the position. This screening includes:

1. Verification of current and past employment,
2. Verification of your highest level of education,
3. Obtaining a state motor vehicle report,
4. Social network and Google media search; and
5. Verification through the US Center for SafeSport.

USA Swimming uses outside vendors, at no cost to you, to complete these screenings, and you will be asked to sign digital consent forms to allow USA Swimming to initiate them. Please provide the below signature to assist with this process.
