Please use the form below to report a Hang Gliding or Paragliding incident you were involved in or witnessed directly within the state of Utah. Please note that near misses are welcome and encouraged as we can learn much from a near miss. You may remain anonymous if you desire, but if you choose to identify yourself it will be very helpful in case questions arise to gain further clarity. This data will be used to identify trends, improve our safety campaigns, and to provide an insight to pilots on areas where we need to exercise caution. Names will never be shared unless you specifically ask that your identity be shared and it is appropriate.
