RHS iPads 18-19: Sophomores, Juniors and Seniors
Welcome Sophomore, Junior, and Senior Families,
 
Parent(s)/Guardian(s) and student must complete this online form together. Upon completion, parent(s)/guardian(s) will get an email from rhsipads@rtsd.org, confirming the form has been received. Note that it may take up to 15 minutes to receive the confirmation.
 
Next steps for parents:
1. Print and sign the confirmation email.
2. Give the printed and signed confirmation email to his/her student.
 
Next steps for students:
1.  Sign the confirmation email.
2.  Bring the items listed below to your homeroom teacher by September 5, 2018:
     a. the printed and signed confirmation email
     b. a $37 check, made out to RTSD Clearing Fund, only if the family wishes to purchase insurance (details in Part 3 of the form).  
 
Insurance and Returned iPads  
For students who returned iPads at the end of the school year, contact the RHS Technology Office after you submit the required insurance form and payment--if opting for district insurance coverage. An insurance form must be returned for all students who have a district iPad, regardless of which option for coverage you select.
