Parent(s)/Guardian(s) and student must complete this online form together. Upon completion, parent(s)/guardian(s) will get an email from rhsipads@rtsd.org, confirming the form has been received. Note that it may take up to 15 minutes to receive the confirmation.

2. Bring the items listed below to your homeroom teacher by September 5, 2018:
a. the printed and signed confirmation email
b. a $37 check, made out to RTSD Clearing Fund, only if the family wishes to purchase insurance (details in Part 3 of the form).
c. a check for the one-time Technology Fee in the amount of $30 to cover the cost of the iPad cover and apps.
Note: One check can be submitted in the amount of $67 to cover the technology fee and insurance cost.
iPad Distribution
Students will receive an iPad in their Social Studies class the third week of school. Students must have their printed and signed confirmation email and a check to cover the technology fee and insurance cost (if purchasing coverage).
