2023 HDSA NATIONAL AWARDS NOMINATION FORM

It’s that time of year when we ask you to nominate the extraordinary individuals, families and organizations in your community for one of our 2023 National Awards. HDSA National Awards embody the very best qualities of our founders, Woody and Marjorie Guthrie, as well as the strength and resilience of the HD community.

Those who receive an HDSA National Award are special; they represent the spirit of our community and the drive to do something about HD. Change begins when one person recognizes that they CAN make a difference!

Who is that exceptional person or organization in your life? Help us to recognize these outstanding community leaders and volunteers who have given so much to your community and to HDSA as an organization.

Please take a few moments to nominate that someone in YOUR community who supports HD families everyday through their words and their actions. Why are you nominating them? What special qualities do they have that sets them apart from others? How have their actions changed the lives of HD families in your community? Tell us about them so we can see them as you do.

All honorees will be announced at the Annual HDSA Convention Gala on Saturday, June 3, 2023 in New Orleans, LA. Additional awards that recognize specific areas of leadership in our Chapters and Affiliates will be presented during HDSA’s Annual Luncheon on Friday, June 2. For National Awards, please take some time to consider the special achievements of any individual in your HDSA Chapter or Affiliate, HDSA Center of Excellence, or community such as a support group leader or advocate, who you think may be deserving of national recognition and complete the online nomination form below.

You may nominate more than one person, or group, as long as each candidate has a separate nomination submission. As part of the nomination process we are asking you to include a photo of your nominee(s) for our Awards Ceremonies. Nominations must be submitted no later than May 5, 2023.

HDSA PERSON OF THE YEAR: The HDSA Person of the Year is an extraordinary individual who is living with HD but does not allow their diagnosis or disability to prevent him/her from serving as a shining example of how to live positively with a chronic disease.

MARJORIE GUTHRIE AWARD: This award honors our founder, Marjorie Guthrie, and recognizes an individual for his/her compassionate leadership and commitment to improving the lives of all persons living with HD both nationally and locally.

WOODY GUTHRIE ADVOCACY AWARD: This award recognizes an individual or group who have given their voice to the fight against HD and who through their activism, awareness efforts and commitment have made a difference for HD families everywhere.

HDSA YOUTH/JHD AWARD: This award recognizes an outstanding youth, who is at-risk, gene-positive or who has Juvenile onset HD, is between the ages of 9-29 and who has, through his/her actions, raised awareness about HD locally, demonstrated leadership and a firm commitment to the HD community.

HD RESEARCHER OF THE YEAR AWARD: This award is given to an individual, or group, who has increased our understanding of the biological causes of HD, whether in the lab or at the patient’s bedside, and in so doing, has brought us closer to finding effective treatments and ultimately a cure for this deadly disease.

HDSA EXCELLENCE IN CARE AWARD: This award is our opportunity to celebrate and honor a care professional or team of professionals, who personify the word “care” through compassion, devotion to our families affected by HD and commitment to our cause.
