AALC 2020 Photo Contest Submission Form

Each year, we look forward to the launch of our Photo & Testimonial Contest. We receive hundreds of photos capturing the true essence of what Supportive Living is all about. We hear stories of what Supportive Living means to residents and family members.  We see the success of Supportive Living in the photos of friends laughing, sharing special moments, playing games and living a healthy, active life. Each entry we receive should demonstrate the important role Supportive Living plays in the lives of Illinois’ seniors and individuals with disabilities while reinforcing what we have always known: Supportive Living = Healthy & Happy Living.

This year, we have all had to adjust our lives due to COVID-19, so your activities and photos may take on a different look than in past years. You’ve created new activities and ways to make your residents smile and feel included in their community. We saw some of the fun you had during Spirit of Supportive Living Week in March, so feel free to enter photos from that week, or add new photos to help us celebrate this year’s Spirit of Supportive Living Photo & Testimonial Contest!


Photo Contest Overview
Your photo entries should capture the Spirit of Supportive Living and demonstrate the many things you do to bring your residents happiness and help them stay active during COVID-19. Ideas include:

  • Residents participating in activities that bring them joy.
  • Residents taking part in arts and crafts, music, dancing and more (SOCIAL DISTANCING MUST BE PRACTICED IN PHOTOS!).
  • Hard working staff members.
  • Touching photos of window visits, etc.
Submit photos using the form below by August 31, 2020.

Winning entries from both the photo and testimonial contests will be awarded cash donations to sponsor resident or employee activities at the Supportive Living communities they represent!
First Place: $150 cash donation
Second Place: $100 cash donation
Third Place: $50 cash donation

Important Note:

All communities submitting entries acknowledge they have a signed waiver on file at their facility stating that the resident agrees to have their photo and/or testimonial entered into the contest and understands it may be used for promotional purposes by AALC on the website, Facebook and in promotional/collateral materials.

Information About You

Photo #1

You may submit up to 10 photographs using this form.  Please be certain that you complete ALL required fields for each photograph prior to clicking SUBMIT at the bottom of the form.  Failure to submit any of the required information will result in that photo being disqualified from the competition.
Your photo MUST be in .JPG format.
Each photograph must be accompanied by a caption and name of all people appearing.
To upload your photo, click "Choose".  
Find the file on your computer.  Then click upload.
When your photo is uploaded, you will see its name listed below the upload box.
