SUSPICIOUS ACTIVITY REPORT (SAR)

CALL 9-1-1 IMMEDIATELY UPON IDENTIFYING A SUSPICIOUS PERSON, BEHAVIOR, OR ACTIVITY

As a reminder, please appoint someone to call the District Office (253-841-1776)
when a 9-1-1 call is in progress.

INSTRUCTIONS: This report shall be completed by the person involved in the incident, the witness, or the manager/supervisor. The completed report shall be submitted to Jake Petersen, CCS Director of Safety & Security, or sent electronically to PetersenJ@CascadeChristian.org as soon as possible but no later than the end of the business day following the date of the incident.

SUSPICIOUS ACTIVITY IS DEFINED AS:

A person seen in an environment where he/she is not authorized; or
A person exhibiting suspicious behavior; or
A person acting and/or behaving in a peculiar way that arouses uneasiness and/or uncertainty.
