INTENT TO ADVANCE EDUCATIONAL COLUMN

NOTE: Appendix A: Section 2: A teacher who plans to advance from one bracket to a higher degree level must notify the Superintendent of Schools in writing (using this form) by December 1 of the year preceding such contemplated advancement.

Please use this form to submit your initial intent as well as to submit your official trasncripts once they are complete.
