If you have read the Participant Information Sheet and associated documents and decided that you are interested in joining the study, please complete the form* below. A member of the ERASE-LC team at your chosen research site will contact you to answer any questions that you may have and check that the study is right for you. We recommend writing down any questions that you may have to ask the researcher.
After the call, if you still wish to take part and the study may be suitable for you, you will be invited to attend a face-to-face screening appointment. At this appointment, we will check you meet the eligibility criteria by asking you some questions about yourself and your medical history before you sign an Informed Consent Form.
*The information that you provide will only be shared with the researchers at the site you are interested in, and with the team at the Peninsula Clinical Trials Unit (PenCTU), University of Plymouth.
