I confirm that I have read and understood the Participant Information Sheet dated 24th January 2024 (version 2.1) provided for this study and have had sufficient time and opportunity to consider this information and ask questions.

I understand that my participation is voluntary, that I am free to withdraw from the study at any time without giving a reason, and that this will not affect my legal rights.

I understand that if I withdraw after two weeks following the interview information already collected about me before I withdraw may be included in the final study after being pseudonymised (personal data changed to a code).

I understand that my personal contact details will be kept confidentially and securely at The Centre for Trials Research (Cardiff University). Both electronic and paper forms will be stored securely at Cardiff University.

I consent to my personal information (for example, my name and workplace contact details) and my interview data being processed for the purposes explained to me. I understand that such information will be held in accordance with all applicable data protection legislation and in strict confidence unless disclosure is required by law or professional obligation.

I agree to the interview being recorded and transcribed by an authorised external professional transcription company and will be handled in accordance with the Data Protection Act 2018.

I agree that my quotes from the interview can be used in any publication of the research findings and that I will not be identifiable in these quotes.

OPTIONAL: I understand that the anonymised information collected about me may be used to support other research in the future and may be shared with other researchers.

OPTIONAL: I agree for my contact details to be retained so I can be invited to take part in later phases of the study.
