I have read the information sheet for this study, ”Dental care for children and young people in care”. I have had the chance to think about the information and to ask questions. I have had my questions fully answered.

I understand that this study will involve the child in my care taking part in an interview either in person or online (using Zoom or Teams) which will be recorded and that the findings of the study, including anonymised quotations, will be published in reports, research journals and presented at conferences.

I agree that if I take part in a focus group I will maintain the confidentiality of focus group discussions.

I understand that the child in my care has two weeks following their interview to withdraw and that their data will not be entered into the study. After the two weeks, I understand that their data will have been anonymised and cannot be separated from the study database. I understand the child I care for can withdraw after they have received the voucher.

I give permission for the anonymised transcript of the interview to be deposited in the University of Plymouth as a digital file so that it can be used for future research and learning.

I agree to the child I care for taking part in the above study.
