I have read the information sheet for ”Plymouth Sunny Smiles”. I have had the chance to think about the information and to ask questions. I have had my questions fully answered.

I understand that this study will involve taking part in a focus group/ interview (delete as appropriate) either in person, on-line (using Zoom or Teams) or by mobile phone, which will be recorded. The findings of the study, including anonymised quotations, will be published in reports, research journals and presented at conferences.

I agree that if I take part in a focus group anything said there by myself or others in the group will be kept confidential.

I understand my participation is voluntary and that I am free to withdraw up to 2 weeks after the interview without giving any reason and without any consequences. I can withdraw even if I have received the voucher.

I give permission for the anonymised transcript of the interview to be deposited in the University of Plymouth as a digital file so that it can be used for future research and learning.
