Providing Vital Statistics and other key information about your loved one prior to the arrangement conference enables us to have the necessary documents prepared in advance. Additionally, it will give you the opportunity to research any information you don't have readily available.
 
Here are the steps in the process...
 
Step One.
Set up your Username / Password so you can save your work and return, if needed.
 
Step Two.
Provide information to determine who has the legal authority to sign the final documents.
 
Step Three.
Complete all the required fields and enter Submit.
 
 
You will receive an email confirming your information has been received by us.

The data you provide is used for filing the death certificate. Make sure all information is complete and spelled correctly.
