I am the parent/legal guardian of the child named above and I give permission for my child to be recorded or photographed whilst at the Spotlight Musical Theatre Academy.
Photographs and video recordings may be used for the following purposes:
Displays at class and concert venues
Show and concert programmes
Concert and show DVD’s and Photo CD’s
Newsletters and publicity material including press articles
The Spotlight website, Facebook and YouTube site.
Children’s names will NOT be given in any publications or online.
